Why many midwives are leaving hospital work behind
The academisation of midwifery training in 2021 marked an important step toward quality and professional recognition. However, the day-to-day reality for many freelance hospital-based midwives remains sobering:
• Often on call around the clock, without permanent employment
• Remuneration only through health insurance funds—with no additional fees from the hospital
• From 1 November 2025: new restrictions under the Midwifery Care Agreement
These structural changes further complicate your already demanding work. Many midwives ask themselves: *“Is this it—or is there something better out there?”*
The alternative: working as a private midwife in postnatal care
More and more families are seeking personalised support in their own homes—provided by qualified, experienced midwives who stand by them during the sensitive period after birth. In this role, you work as a maternity nurse or private midwife in postnatal care—individualised, self-determined, and meaningful.
Typical areas of work:
• Comprehensive postnatal care (4–12 weeks), 6 days a week, including overnight stays in the family’s home
Night Nurse: night shifts from 7:00 p.m. to 7:00 a.m.
Day shifts: flexible care on 5 days per week
Remuneration is paid directly by the families—independent of health insurance providers. For you, this means planning security, appreciation, and a working environment in which your work is truly valued.
Fees on a self-employed basis:
• Day rates starting at approx. €500 gross (24-hour care)
• Up to €1,000 per day with relevant additional qualifications or references
